• Abstract:

  The effectiveness of osteoporosis treatment is declining, especially in patients with fractures. New, low-cost interventions are needed to improve osteoporosis. We conducted a parallel, controlled, randomized clinical trial to evaluate a behavioral intervention to improve the use of osteoporosis drugs. A total of 2684 women did not use the osteoporosis intervention of the global women's osteoporosis longitudinal study (glow) after the age of 45 and had a history of fracture. They were randomly divided into 1:1 to receive multimodal, customized, direct video intervention and routine care for patients. The main findings were self-report of osteoporosis drug use at 6 months. Other outcomes included calcium and vitamin D supplementation, bone mineral density (BMD) testing, behavioral change preparation, and intervention disorders. In the intention to intervene analysis, there was no significant difference between the intervention group and the control group in the use of osteoporosis drugs (11.7% vs 11.4%, P ¼ 0.8), calcium supplement (31.8% vs 32.6%, P ¼ 0.7), vitamin D intake (41.3% vs 41.9%, P ¼ 0.8) and bone mineral density test (61.8% vs 57.1%, P ¼ 0.2). In the intervention group, many women responded to concerns about osteonecrosis of the jaws and difficulty taking / remembering osteoporosis drugs. We found differences in BMD tests among women who did not receive osteoporosis intervention, those who provided contact information, and those who did not receive osteoporosis examination. In the protocol analysis, women with significant exposure to online interventions (n ¼ 257) were more likely to start taking non bisphosphonates than those in the general care group (odds ratio or ¼ 2.70; 95% CI 1.26-5.79). Although our intervention did not increase the use of osteoporosis intervention at 6 months, it increased the use of non bisphosphonates and bone mineral density test, changed the participants' preparation for behavior change, and changed the understanding of osteoporosis intervention obstacles.
